YOURSUPPORTNURSE                                                                                 This service begins with an initial analysis and action plan developed by your Nurse Care Manager.                                                                                               She will:

• Help you understand your parent/loved one's medical condition and what to expect
• Arrange for local nurses to visit your parents and help ensure their independence at home, at an assisted living facility or nursing home
• Prepare an individualized action plan and coordinate solutions for and with you.
• Provide a customized family web portal to keep family members informed of progress

Philips Lifeline Medical Alert Service
Lifeline is an easy-to-use medical alert service designed to reduce the risk of living alone.  In the event of a fall or emergency, help is available at the push of a button.  The Lifeline Personal Help Button connects you to a trained Personal Response Associate who can send help quickly - 24 hours a day, 7 days a week. 

Philips Lifeline MD2 - Reduce risk of medication errors with Lifeline MD.2.      MD.2 works by dispensing the medications you've pre-loaded, at the times you've scheduled.  This hellps ensure you get the right dose, at the right time-so you and your family don't have to worry about complications from incorrect medication use. 

Medicaid Waiver Programs - Participating Provider

SOURCE Partners Atlanta - (Service Options Using Resources in Community Environments) creates a continuing care network for at-risk individuals who need medical attention and personal care and prefer to stay in their own homes and communities.  This alternative to nursing home placement provides comprehensive, preventative medical care along with in-home support services.  Case management is central to identifying and providing appropriate services. How to Apply - For eligibility and services call 404-463-3248.

Community Care Service Program (CCSP)                                                      Right at Home participates in the Community Care Service Program providing Personal Support/Extended Personal Support services. The Community Care Service Program provides services to people who are functionally impaired to live dignified and reasonably independent lives in their own homes and communities. It provides home and community-based Medicaid services to nursing home-eligible consumers. The CCSP has successfully served eligible consumers in Georgia for 21 years.

The Division of Aging Services (DAS) administers the CCSP in coordination with the Department of Community Health (DCH).
